ABSTRACT

According to literature reports, the injury rate of the athletes in Olympic Winter Games recent years was as high as 10%-14%. Combined with the background of corona virus disease 2019 (COVID-19), the medical insurance work of the 24th Olympic Winter Games held in Beijing had put forward more complicated requirements and more severe challenges. In order to better optimize anesthesia management, this article summarized the perioperative treatment of athletes in Olympic Winter Games, the safety protection strategy of medical staff under general anesthesia, and the potential impact of peri-operative drugs on athletes. Anesthesiologists, as the core members of the rescue team, should be familiar with the particularity of operative anesthesia of athletes, sum up relevant experience to ensure the safety of perioperative patients. So all kinds of technical measures should be taken in the process of operation to minimize the indoor pollution caused by the patient's cough. For example, all the patients should wear N95 masks from the ward to the operating room, and after the operation, wear the N95 masks back to the ward. Although the International Olympic Committee had banned more than 200 drugs for participants and athletes who had to strictly follow International Olympic Committee requirements during anesthesia, the athletes were no longer participating in this Olympic Winter Games, so opioids (sufentanil and remifentanil) and glucocorticoid (dexamethasone) could be used according to the actual needs of surgery and anesthesia. Five athletes in Yanqing competition area underwent surgical anesthesia in Peking University Third Hospital Yanqing Hospital. All the five patients received general anesthesia, of whom four underwent orthopaedic surgery and one underwent laparoscopic cholecystectomy. General anesthesia with laryngeal mask airway was the first choice in the five patients. And the pain after orthopaedic surgery was severe and nerve block technique could effectively relieve the pain after surgery. Three patients received ultrasound-guided nerve block analgesia, the postoperative analgesia lasted 36 h. After the operation, non-steroidal anti-inflammatory drug (NSAID) was infused intravenously in the ward and all the patients recovered uneventfully. As the core member of the trauma rescue team, anesthesiologists should be familiar with the particularity of the athletes' surgical anesthesia, do a good job in medical security, and summarize relevant experience to ensure the life safety of the perioperative patients.

ABSTRACT

Domestic tourism plays a crucial role in the Australian economy, generating revenue, creating employment opportunities, fostering cultural identity, and facilitating tourism growth and development. The remote regions of Australia are particularly reliant on domestic inbound tourism to stimulate their local economies. This study investigates the influence of heritage sites and various factors on domestic tourism inflows to eight states in the Australia between 1998-2021. The gravity method and random effect model are employed for the empirical analysis. The results indicate that the macro determinants, including population of origin state, gross state product per capita, infrastructural development, shared border between states, and the number of heritage sites, have significant and positive impact on domestic tourism inflow. Conversely, the consumer price index, distance, and pandemic outbreak have a negative influence on domestic tourism inflow. These findings hold important practical implications. Given Australia's geographical remoteness, promoting domestic tourism becomes imperative to boost the tourism industry and local economies. Therefore, it is recommended that authorities prioritize domestic tourism flows and invest in infrastructure, preserve heritage sites, stabilize prices, implement effective marketing strategies, and respond swiftly to public emergencies such as the Covid-19 pandemic.

ABSTRACT

Between 2018 and 2022, a total of 231 PM2.5 samples were collected in Nantong, China, and analyzed online for 16 US EPA PAHs utilizing a variety of analytical methodologies, including high-performance liquid chromatography data analysis, principal component analysis, and characteristic ratio analysis. The seasonal variation and pollution characteristics of 16 PAHs in PM2.5 over a long period in Nantong, China, were analyzed, and correlations and traceability changes with a variety of emission sources were established. Additionally, health assessment models for BaP equivalent concentration, daily exposure dose, lifetime excess risk of cancer, and life expectancy loss were constructed for various ages and genders. The results indicated that the total number of days with BaP concentrations less than the national limit standard of 1.0 ng/m(3) was 60.17% (139/231), with rates of 25.40%, 35.71%, and 94.05% from 2019 to 2021, respectively;the total concentration range of 16 PAHs demonstrated a downward trend, but pollution was relatively severe in winter. The seasonal variation showed winter > autumn > spring > summer. The detection rates of 4 and 5 rings were generally high and fluctuated between dropping and increasing. Principal factor analysis and characteristic ratio traceability analysis indicated that PAH pollution is primarily caused by local coal-fired sources and traffic emissions, with a new trend of biomass combustion. Furthermore, BaP remains the primary contributor to carcinogenic factors, and the health risk is higher in females than in males, and in adults than in children. A low level of PAHs in the air may have health benefits. Traffic control and flow restrictions, as well as production restrictions, are all part of Nantong's COVID-19 pandemic prevention and control policies. These policies contributed to PAH risk prevention and control, as well as pollution reduction.

ABSTRACT

Objective  To describe the spatial distribution of COVID-19 cases in Shaanxi Province and further explore its relevant factors, so as to provide evidence for the prevention and control of COVID-19 in Shaanxi Province.  Methods  The information of confirmed COVID-19 cases and relevant socioeconomic data in Shaanxi Province were collected. The temporal and spatial distribution characteristics of confirmed cases, and the correlation between the incidence of COVID-19 and socioeconomic factors in the population were analyzed by using a generalized linear model.  Results  Four cases were first reported in Shaanxi on 23 January 2020, with the highest number of new confirmed cases reaching 23 on 4 February and no new cases after 19 February. The imported cases appeared earlier and reached the new peak than the local cases, and entered the zero stage earlier than the local cases. The spatial distribution showed that Xi′ an (120 cases) had the largest number of confirmed cases, accounting for 48.98% of the total cases, and the districts with more confirmed cases were in Lianhu, Yanta, Xincheng and Weiyang. Socioeconomic factors which significantly associated with the number of confirmed cases in each district and country were education expenditure (IRR=0.287, 95% CI: 0.134-0.612), GDP per capita (IRR=1.143, 95% CI: 1.049-1.245) and the distance from Wuhan (IRR=0.995, 95% CI: 0.992-0.998).  Conclusion  Measures should be taken in key areas and population at the early stage of the epidemic to control the spread of the epidemic as soon as possible. © 2021, Publication Centre of Anhui Medical University. ABSTRACT

Automatic segmentation of lung lesions from COVID-19 computed tomography (CT) images can help to establish a quantitative model for diagnosis and treatment. For this reason, this article provides a new segmentation method to meet the needs of CT images processing under COVID-19 epidemic. The main steps are as follows: First, the proposed region of interest extraction implements patch mechanism strategy to satisfy the applicability of 3-D network and remove irrelevant background. Second, 3-D network is established to extract spatial features, where 3-D attention model promotes network to enhance target area. Then, to improve the convergence of network, a combination loss function is introduced to lead gradient optimization and training direction. Finally, data augmentation and conditional random field are applied to realize data resampling and binary segmentation. This method was assessed with some comparative experiment. By comparison, the proposed method reached the highest performance. Therefore, it has potential clinical applications.
